Why Tracking referral source is useful:

  • Ensure safe and appropriate patient care (knowing why and from whom the referral came).

  • Support continuity of care and communication with referrers.

  • Meet NHS, CQC, and governance requirements (audit, DAF indicators).

  • Track how patients access the practice (service development, marketing).

  • Maintain a clear medico-legal record of referral history

What Report tells me:

  • Referral Type

  • Referral Source

  • Referral Description

This information is found by running the New Patient Created Analysis Report found under the Patient and Clincial tab:

  • Select required date range

  • Select Practice(s)

  • Click Create

🚨 Important: Use scroll bar at the bottom of the report to scroll far right to see Referral Data.

Referral Type


Indicates the category of referral recorded in the Patient Management Software (PMS) under the patient details screen.

  • Ref Source – Patient: referral made by another patient.

  • Ref Source – Other: referral from a different source (e.g., GP, organisation, NHS pathway).

  • If no referral source is recorded, this field will be blank.

Referral Source


Shows the code of the referral source, depending on the Referral Type.

  • If Referral Type = Patient, it displays the code of the referring patient.

  • If Referral Type = Other, it displays the code of the external source (e.g., SIGN, GP code, organisation code).

  • Codes are selected from the PMS referral source list.

Referral Description

Displays the name or description linked to the referral source code (e.g., “Practice SIGN”).

  • To download a copy of your report you can use the Export Data button to download a CSV spreadsheet.

  • You can use the Print button in the top right as needed.

  • To set up automation of the report you can use the Schedule Report button.
